Wiki Wellcare denial

I received a denial statement from Wellcare stating onset of current illness or symptom should not be used. What does this actually mean? The diagnosis codes are 577.0, 288.60, 276.8, 574.20, 157.0, 401.9. My doctor was the patient's attending.

It does look like you have over coded the visit. I could better advise if I saw the note. But you usually do not see pancreatitis and pancreatic cancer coded at the same time for instance.
