Hello, I wanted to get some clarification on confirmation of pregnancy billing. Sometimes we have patients who schedule their well woman exam a few months in advance, between that time they conceive and miss their period. They have not been to any medical facility for a confirmation and may only have signs due to the missed period. If the provider performs the full well woman exam and also does a urine pregnancy test (Z32.01 - positive pregnancy test) which is positive, should the appointment type be changed to a 99213 due to the positive pregnancy test/confirmation of pregnancy? Or can we bill the annual and the pregnancy test showing positive? I feel that the latter is appropriate as she was originally scheduled for the annual anyways. Thank you in advance for any help!
 
Most agree the intent of the visit was an annual exam. The annual should be coded as such with an additional diagnosis for the positive pregnancy test. You should be able to find documentation from ACOG supporting this billing scenario. Supercoder has an article this month addressing when to begin ob records. This was included.
