Wiki well vs sick - everything old is new again!

I have a doc who is charging a sick visit at almost every well visit. Her contention is that if a patient has a condition that is brought up during the visit (possible a med check/refill), she has the right to charge a sick visit. Patients are upset because they have balances (copays, etc.). Are there guidelines to address when a sick visit is appropriate to bill with the well visit? I have read the July and September, 2016 articles again, but need real guidelines to present to this physician.

I've read that it would be appropriate to bill the sick portion if the patient would have come in anyway for whatever the complaint is. For instance, if it is found they have a severe sore throat and get diagnosed with strep, then I would consider that to be appropriate. But if your provider is charging a sick visit for the patient having a cough, that is excessive.
