If a patient completes treatment and has one or two fractions left - you can't bill for those. What happens if three or 4 months later the same pt is back in with metastasized ca to a different location and has treatment. Can you count those leftover fractions on the new treatment?
 
The treatments themselves are billable. The physician treatment management code (77427) is not billable when there are less than three fractions in the final treatment period for the course. You cannot combine a separate course which is started months later to a previous course to bill the physician treatment management code. The physician management code is per the course of therapy.
