I am looking for CPT codes to use for this Vivaer procedure. The rep says to use 30117 but I don't feel this is an accurate code.
 
30999 is used if the doctor is remodeling the upper nasal valve
30117 is used for the destruction/ablation of the septal swell bodies. If swell bodies on the other side are also destroyed, code it 30117-59 (or XS if the payer recognizes the “X” modifiers) because 30117 cannot be coded bilaterally
30801 is used for the submucosal reduction of the inferior turbinates. Even though the low temperature radio frequency reaches the submucosal level, there is no incision, so 30802 cannot be coded.

Question: Nasal swell body (NSB) reduction was performed on septal mucosa during the same session as septoplasty. Is it appropriate to report code 30117 separately in addition to code 30520?
Answer: No, it is not appropriate to report code 30117, Excision or destruction (eg, laser), intranasal lesion; internal approach, separately for nasal swell body reduction. The procedure described in code 30117 is included in code 30520, Septoplasty or submucous resection, with or without cartilage scoring, contouring or replacement with graft, and should not be reported separately.
Question: Nasal swell body (NSB) reduction was performed on septal mucosa during the same session as inferior turbinate ablation. Is it appropriate to report code 30117separately in addition to code 30801 or 30802?
Answer: Yes, code 30117, Excision or destruction (eg, laser), intranasal lesion; internal approach, may be reported in conjunction with either code 30801, Ablation, soft tissue of inferior turbinates, unilateral or bilateral, any method (eg, electrocautery, radiofrequency ablation, or tissue volume reduction); superficial, or 30802, Ablation, soft tissue of inferior turbinates, unilateral or bilateral, any method (eg, electrocautery, radiofrequency ablation, or tissue volume reduction); intramural (ie, submucosal), when used to report NSB reduction. The destruction of nasal lesions is not inherent to either the superficial or intramural inferior turbinate ablation. It is important to note that, if a septoplasty (30520) is reported with code 30117 (to reduce septal mucocal NSB), in addition to code 30801 or 30802, then the procedure described in code 30117 would be inherent to code 30520, and therefore, it would not be reported separately.
