Wiki Visual field testing

Does anyone know how to bill a visual field with a dx of plaquenil..i have tried using the v58.69 and it keeps getting kicked back..any help would be greatly appreciated

You must use a second DX which indicates why the patient is taking the medication. This is usually used for arthritis. Hope this helps.

2nd dx V58.69 (current use medication)
OR V67.51 (no longer using high-risk medication)
3rd dx Reason for using drug / ie, rheumatoid arthritis 714.0
4th dx E-code for drug
