Wiki Venipuncture 36415 Documentation

I'm looking for guidance on what documentation is recommened for CPT 36415. In our clinics we are going to have MAs doing blood draws and I've been asked to come up with a template for them to document this service... anyone have suggestions on what needs to be documented/what you are doing for documentation?
