"Previous chest tube site was irrigated with Betadine. This was used as a port. Upon entry with the thoracoscope, there was significant old clot and we were able to clear a space to put a second 10 mm port in the posterior axillary line eighth interspace as well as a ninth interspace 5 mm port.
Using this combination of ports, the hemithorax was evacuated with approximately 1 liter of old clot and blood. All the areas were explored to assess for more bleeding. There was none. The patient had no evidence of studding on his pleura. All pockets were cleared. The chest was irrigated with 3 liters of warm sterile water. A 28-French chest tube was positioned through the most inferior ports and secured with 2-0 silk sutures. The remaining port sites were closed using 2-0 Vicryl for the muscular layer, 4-0 Monocryl subcuticular for the skin. Dermabond was used as a dressing.
