From the documentation below, what ICD-9 would one select, or would it be two/three code combination?
"Moderate hemodilution, moderate hypothermia, cold hyperkalemic blood cardioplegic arrest delivered antegradely and retrogradely, and Amicar. Also utilized was 1.5 mg/kg of methylene blue at the beginning of the procedure and later methylene blue was given at 1 mg/kg near the conclusion of the procedure. The valve utilized was a 21 mm On-X Conform-X cuff valve."
"The patient required considerable volume replacement and he did have vasoplegia at the conclusion of the procedure."
