patient had mitral valve replacement 6 months ago. now he returned for pulmonic valve replacement and tricuspid valve repair. would i code 33530 with 33464 and 33475 or dont code 33530

As long as it's one month after original operation, then yes. It's for use with codes 33400-33496 which both codes fall into.
