Wiki Valid CMS charting

Our department needs to perform some indepth instruction to our PCPs as to what CMS considers valid chart documentation to meet RADV audits. Can anyone tell me where, in the CMS literature, I can locate these documentation guidelines? We want to provide concrete evidence directly from "the source."
