If you were reading a colonoscopy report and the doctor dictated;

Indication for procedure: History of prior colorectal polyps

Would you use V12.72 for your diagnosis or would it specifically have to say; History of prior adenomatous polyps?
 
V12.72 is for personal histroy of colonic polyps. So yes, V12.72 works in your situation.

However, for high risk screening most payors that I've dealt with want the personal history of polyps to be personal history of adenomatous polyps.
