Wiki Use of Consent for Surgery forms

I work as biller/coder for an internal medicine doctor. I just graduated and took over this job from the previous biller/coder who retired. My question is should we be having the patients sign a consent for surgery form when joint injections and office surgeries are done? They have never done that and I believe they should. Does anyone have some input on this?
 
I was recently at an audting training session when a question arose of this nature. The instructor informed us (potential auditors) that if a procedure requires the skin to be broken/compromised, a consent should be obtained. Hope this is useful.
