I'm a bit confused when the "Use Additional Code" indication is next to the dx; yet there's no subclassification key for that particular section to place the specific digit as it relates to that disease. How can I know what the last digit should be if there's no key to further describe the disease?

I am confused by your question. The use additional code notation is very different from the code requires additional character notation. Also each book publisher uses different symbols to note that additional characters are required. Can you provide an example of a specific code and what exactly you are seeing in your reference book.
