Our doctors are going to start doing the Urolift.
They will be doing a Transrectal Ultrasound in the office prior to the procedure for mapping.
What code do we bill for the Ultrasound? 76872?
As for diagnosis code are you just using Elevated PSA or BPH? Is there any additional dx code?

Yes that is correct for the ultrasound. Some insurances require certain documentation prior to doing procedure. BCBS is very picky- Just a heads up!
