I work for a neurologist who does a lot of treatment for drug addiction/abuse. He screens UA to see if they have Suboxone in their system and no other opiates. We would bill out F11.20 for the premlinary screening done in the office. Wanting to know if this is correct because the ICD 10 book says F11.20 is for opiate dependence and F11.10 is for opiate abuse. I would also bill out Z79.891 along with the F11.20. The CPt code is G0434 or 80300 for commercial payors. The ICD 10 book states for the Z79.891states to not bill this and to bill out F11.20 for heroin abuse, so now I am confused. The doctor puts in the notes patient is coming in for chronic opiate abuse. Some of these patients have been off the opiate for a long time, they need to pick up more Suboxone and so then I was thinking the diagnosis should be F11.21 opiate dependence in remission.
