Hi, I'm billing for UDS 12 panel in Pain Management Office, but we are having issues with reimbursement from multiple insurance companies including Medicare I'm billing G0434 modifier QW with a DX code V58.69. Is anyone else having this issues? Anyone know a better way or different codes that I should be using to be reimbursed correctly.

What are your denial reasons? It may not have anything to do with your choice of CPT code, but many factors can lead to denials of drug testing.
