have a blue cross/blue shield patient (also has medicare as secondary) with bilateral venous stasis ulcers of both lower extremities. Putting Unna boots on weekly. Claims rejected, no reason. I've tried a 50 modifier, list rt and lt separately, neither works. any suggestions on how to code?

Usually appending modifier for LT or RT (whichever the case may be) usually works. You might need to take a second look at the EOB. The insurance should list a denial reason. If indeed no reason is listed, then I would recommend you call the insurance company to find out the reason. Or, with BCBS of VA, we can submit a 151 form online through the portal to inquire about a claim or submit additional info, etc.

I've tried to call the insurance company today but they are closed due to the weather. I'm going to wait till next week and maybe I'll have better luck then. Thanks for the reply.
