Why do claims get rejected when we assign unlisted HCPCS codes, especially L8699 for orthopedics implants (specifically hip joint and spinal implants that are implanted during major procedure)?
 
What all additional information that is required to be submitted to the insurance so as to avoid claim rejection. It would be highly appreciated if any of us could share a thought.
