Does anyone have any tips for getting unlisted procedure codes paid in the CAH - Method II billing setting where professional fees are billed on a UB04? Our contractor is denying saying they cannot accept unlisted codes on a UB04. We are specifically struggling with 37799 - vein therapy, 28899 - tobaz fasciotomy, and 64499 - ganglion impar block and a few others. Thanks!
 
Hi! Did you ever get any traction on this? Having the same issue and I'm not seeing that it's really allowed given the disallowed revenue codes per Noridian's Outpatient Billing Guide for CAH. 96x, 97x and 98x are disallowed which means no profees....
