Wiki Unlisted code bundled by medicare

Is it typical for MCR to bundle an unlisted code with whatever other procedure is being billed at the same time? I used 28899 for an arthroplasty of MTP join lesser digit with the fee comparable to 28293. MCR bundled it with a hammertoe procedure.

Seems like I always had to appeal those because they need the notes. I also included a letter explaining why I thought it should be priced a certain way.
