We're a Workers' Comp payer and we received a bill for heparin lock flush that had this information:
100 usp units/ mL , 5 mL fill in 10 mL pre-filled heparin lock, flush syringe. (NDC #08290-3065-13) (30/sp, 120/ca)

They billed for 1 case (120 syringes) and they charged around $84. The description for
J1642 states "per 10 units." They billed J1642 x 120 which doesn't seem correct--the fee schedule (Ohio) is $.19 per unit so they only got $22.80. Can anyone explain the correct way to calculate the units for this?
