What code do you recommend?

Patient had a prior unicompartmental knee arthroplasty.

Is now going to a Total knee arthroplasty.

Is that considered a revision ? 27487?

I am thinking yes, but would like confirmation.

This is a very interesting situation and would be interested in others' opinions as well. I do not think that I would use 27487 because that is for revision of a total, which is not the case here. I would be more inclined to use 27447, possibly with a 22 modifier for removal of the unicompartmental hardware.
