Scenario:
A midlevel provider, such as nursepractitioner sees a patient for an officevisit and the patient has an ultrasound on the same day performed by an ultrasound technician.
United Healthcare does not allow contracting for nurse practitioners, and they dictate the nurse practitioners services must be billed incident to (under the physician's NPI) with an SA modifier.
The physician signs off on the encounter and reviews the visit. The physician also interprets the ultrasound report, and own the ultrasound equipment. (the sonographer is a contractor working for the physician)
Is is appropriate to bill both the services (officevisit and ultrasound) as incident to?
Or should the officevisit only be billed as incident to, and the ultrasound be billed on a seperate claim under the physician only?
