Wiki Ultrasounds and Xrays

Hello Coders,

What documentation requirements are needed for Ultrasounds and Xrays? Are the images REQUIRED to be saved in the patient's records in order to bill for it? Or is the interpretation/findings adequate enough? Or are both required?
