New codes are coming in 2015 for ultrasound. What type of documentation is needed as far as a "blurb" for our EMR to support this code? (ortho)

All radiology needs a separately documented radiology report in order to report the professional component, a "blurb" in the EMR is insufficient. I worked with an ortho clinic that use to document as a blurb the results and during a routine audit, Medicare did not recognize this a radiology interpretation, they went back 7 years and recouped all monies paid for radiology.
