I received a denial on cpt 93880 from BCBS Medicare stating the dx was not covered so I appealed it with our LCD's showing it was a covered dx for that procedure. I received a status letter stating the homeplan (this is an out of state BCBS medicare advantage plan) has advised that 93880 was denied per NCD guidelines and that it has not met the relationship criteria per CMS 220.5. I reviewed the NCD but am unable to get clarification. Does anyone have any insight on this? It would be greatly appreciated. Thanks
