when billing for CPT code 62290 (Discogram) x 3 levels in an ASC, would you list the PPC code on the UB 3 times, or is it listed just once and it represents all three levels. In this instance, this is a workers comp claim. I know that mostly when done in an ASC the reimbursement usually seems to be a set rate but seems if there were multiple levels of this procedure done that would be worth additional reimbursement. I'm new to ASC billing and coding and not sure. I know on the physician side each level is reimbursed. Off the top of my head I think the PPC is 03.09. Any thoughts would be greatly appreciated.
