Hello, I've started having problems getting medicaid(soonercare) and medicare to pay for UA's. I file as follows I just can't get them paid

81002-QW

DX's we have used without success: UTI, Dysuria

I have had the same issue with the QW modifier

I have had this issue try taking off the QW

Looking at the Medicare clinical laboratory fee schedule I don't see a QW which could be why its rejecting.

*Edit* Here is the source from Medicare. The code is on table 2 for CLIA waived tests that do not require QW modifier

Refiled claims

I refiled the claim without the QW and still denied.
