How would you handle this?
Ortho Surgeon takes patient to have a toe amputation and a fasciotomy of the posterior compartment.
During the fasciotomy, it is discovered that the patient's infection, and muscle tissue has severely necrotized and they have to amputate the leg from below the knee.
Another surgeon from a different specialty comes in to amputate the knee.

Co-surgery rules state that if surgeons of different specialties are each performing a different procedure (with specific CPT codes), neither co-surgery nor multiple surgery rules apply (even if the procedures are performed through the same incision).

It doesn't seem appropriate to bill for a knee amputation (most extensive procedure) as a co-surgery, however, how would you bill for a toe amputation and fasciotomy on a leg that was eventually amputated in the same surgical session?

Any thoughts?
