I had one of my docs pose the following questions regarding Modifier 22:
1. Can Mod 22 be used on multiple codes performed during the surgery if the circumstances making the surgery more difficult (e.g. morbid obesity, difficult approach) affected the entire surgery (e.g. 63045-22, 22600-51,22)?
2. Is it true that the application of Mod 22 should not exceed 5% of a docs total surgeries or it will trigger an audit?
