Wiki Two physicians, two hospitals, same practice, same patient

How do you appeal when the ER visit is denied when the patient is first seen by Dr. #1 in Hospital A, stabilized and transferred to Hospital B where he is evaluated and treated by Dr. #2. The two doctors are in the same group practice and both E&Ms were high level.

Same specialty same practice is treated as one provider,you can bill only one level.
