Looking to see if most are billing 99211 for the reading after placement. Insurance denies for preventative diagnosis as maximum allowance if well visit done in the same year.

The reading is included in the administration of the test. You shouldn't be billing for anything for the reading.
It's been a long time since I worked primary care, but we never billed for reading if we placed the test. We would bill 99211 in the rare instance we were reading someone else's test (which involved additional documentation and work).
