"Now a mixture of 1cc of 40mg/mg triamcinolone and 1cc of 1% lidocaine was slowly injected into the epidural space."

Diagnosis:
Lumbar Radiculitis

Procedure:
1. Left L5-S1 Transforaminal Epidural Steroid Injection
2. Fluoroscopic Guidance

Details of Procedure:
The patient was placed on the fluoroscopy table in the prone position using a pillow under the pelvis for positioning. The skin overlying the sacrum and low back was then prepped and draped in the usual sterile fashion.

The L5-S1 spinal interspace was identified by fluoroscopy. AP fluoroscopic viewing was then used to align the endplates at the L5-S1 level, and the C-arm was rotated to the ipsilateral side to allow proper entry under the L5 pedicle. I then inserted a 25-gauge, 3 1/2-inch spinal needle in-line with the beam. The needle was advanced anteriorly to a location just below the inferior aspect of the left L5 pedicle, just superior to the exiting nerve root. Lateral fluoroscopic imaging was then used to confirm adequate needle depth, just below the pedicle. The introducer needle was then removed, and the needle was observed for revealing no CSF drip back. A 6-inch, low volume extension tube was then attached to the needle for the injection. First, a small amount (1-2cc) of Optiray 300 contrast was injected, demonstrating perineural and epidural flow. Now a mixture of 1cc of 40mg/mg triamcinolone and 1cc of 1% lidocaine was slowly injected into the epidural space. The needle was then withdrawn, and a sterile dressing was applied to the injection site.
