I need some help clarifying a scenario. If a patient is discharged on a Friday and our office is closed and he or she cannot schedule a follow up appt., can the doctor still do the transitional care since there was no communication within 2 days of discharge? Also, same scenario and let say he or she was discharge again on a Friday and could not be seen until 8 days later. Since there is a weekend included in the 8 days will that make the transitional care code 99495?
