Should one think of Transitional Care Management like Fracture Care? For example, if you bill TCM and within the 30 days the patient is seen in follow up for the TCM problem you can NOT bill an additional E&M. However, if the patient needs to be seen during the 30 days for a different problem you CAN bill an additional E&M.
