We see post hospital stay patients within 2 weeks. We have been using the office visit codes 99213-99215. Can we use transitional care management codes 99495 instead? What is the difference in documentation requirement over an SOAP office visit notes?

Does the patient contact documentation need to be in the notes or just in the chart?

This link will provide the info you need to bill TCM codes 99496 & 99495. Check with your commercial payors to see if they can identify the code. BCBS does NOT pay TCM, but a lot of payors do.
