Is anyone billing the new transitional care code 99495, and if so are you getting paid?:rolleyes:

I'm also curious about this. Has anyone been paid for these services? How are you contacting the patient with in 48 hours of discharge? And, is your provider also following the patient in the hospital? I'm trying to figure out how to capture this information.
