Where can I find coding/billing info for Transitional Care Codes?
I have searched the Medicare website some, but need more specific information on exact(ness) of date requirements & also definition of both communication with (patient) & documentation required, etc.
We are Family Practice Providers in Louisiana and these claims are only being paid sometimes (we are mainly billing Medicare & Medicare Advantage Plans, but do have patients with Commercial Insurance)
Need clarification for both Clinical & Billing departments
