I believe but looking for reassurance when TOC is done meeting all key elements the Provider should code for the reason for the hospitalization even if that has resolved IE COPD exacerbation

When you say transition of care, do you mean the person is following up with their PCP or outpatient specialist? If that's the case, no, you would not code for the resolved condition. Outpatient coding should only include currently active conditions (or appropriate history codes.)

If the patient was hospitalized for an acute complication of a chronic condition (diabetes w/hyperglycemia or exacerbated COPD), you can still code the chronic condition, just without the complication. If it was something like chest pains or dehydration that's now gone, I would use Z09.
