What code would apply to the scenario below?

PREOPERATIVE DIAGNOSIS: Chronic patellar tendinitis.

POSTOPERATIVE DIAGNOSIS: Chronic patellar tendinitis.

PROCEDURE PERFORMED: Topaz treatment, patellar tendinitis.

DESCRIPTION OF PROCEDURE: Under general anesthesia, an incision was made over the patellar tendon and carried down through the subcutaneous tissue. Bleeding was controlled by cautery. The paratenon was visualized and split throughout its length. The patellar tendon was exposed and using a Topaz wand , multiple holes were made into the tendon in a rectangular type fashion. Upon completion, the wound was irrigated, the paratenon was reapproximated, the skin was closed with 3-0 Vicryl and 3-0 Monocryl. The patient tolerated the procedure well, a compression dressing was applied, and the patient was taken to the Recovery Room in good condition.
