Can someone help me code this. Do I use 11730 and 11740 or do I use 11740 and 11750?

Complete toenail removal performed along with evacuation of subungual hematoma.
Wound was dressed. Instructed patient to remove dressing in 24 hours and to do
warm soapy water soaks for 15 minutes daily followed by gauze dressing. Note was
written to be off work for the next 7 days as patient works in steel toe
boots. Medication for pain was given as needed. Also may use ibuprofen. Given patient
type I diabetic high risk for infection prophylactic antibiotics were also prescribed. Patient
to follow-up if increased redness, pain, fever or purulent discharge

Sterile prep performed right second toe
1 cc 1% lidocaine injected locally into the paronychia. Incision was made draining large amount of blood.
Subsequently 2 cc 1% lidocaine then injected digital block to the right second toe base.
Toenail was then completely removed using forceps.
Wound was cleaned using sterile saline and sterile gauze.
Compression dressing was applied. Procedure tolerated well without complication
 
11740 is used when the hematoma can be relieved by placing a small hole through the nail bed allowing the hematoma to drain (trephination). Since the entire nail was removed to relieve the hematoma the appropriate code would be 11730. If this was the initial visit for this condition, add an appropriate E&M.

The 11750 is used when a chemical such as phenol is used to kill the matrix of the nail.
