The right foot wound on the planter aspect of the distal 4th and 5th metatarsal was excisionally, sharply debrided to remove all non-viable dermis, tissue, muscle, tendon, bone using a scalpel, scissors and forceps. An oscillating bone saw was used to remove the distal metatarsal head and shaft for the 4th metatarsal to healthy bone proximally. Proximal bone culture was sent. Wound measurements post-debridement were 7x5x2 cm.

Did the doctor call this an "amputation"? If the surgeon performed an amputation the code would be 28810. If the surgeon just removed bone from the metatarsal it would be 28122. The excisional debridement area is large, it would cover about 1/4 of the dorsal foot. If the toe was amputated and if the debridement was performed in the same area to prepare for amputation it would not be reported separately. My guess is that this debridement covered much more area than just a single metatarsal.
