provider decided to do a TKA after getting into the knee area, but needed to remove some hardware. Following is the statement in the OP note.

"The two coventry staples interfered with precise placement of the tibial components. They were therefore identified and
exposednusing a small osteotome. Surrounding bone removed and the staple removed intact."

I coded a TKA 27447, but can I or should I code a hardware removal. I also don't feel this is a conversion. Can someone please help me with this.

Jeanne:)

The hardware removal is included in the procedure as the surgeon has to remove it in order to perform the TKA. The provider was in the same incision at the same body area and the two codes bundle so adding a mod-59 to bypass the NCCI edit would not be correct.

Now if hardware removal was on the contralateral side, yes, you could bill it and support the mod-59 use.
