Wiki Tissue Markers, implantable - kits

We purchase tissue markers that come in a pack of 3. However we are going to use them in a cyberknife procedure in which they place 4 markers. Can I bill for 2 kits? I use A4648 which just states "implantable, any type, each" except my "each" is 1 pack of 3. I hope I am making sense...

Each means each individual tissue marker. So its 4 units if you use 4 markers regardless of the pack size.

I'm not form the AAPC other than being a member, but the HCPCS book states A4648 - Tissue marker, implantable, any type, each. Bolded emphasis, is mine.

does anyone know the rules for billing tissue markers for medicare patients? I just read an article by Novitas dated 4.14.2016 mentioning we need to list the acquisition price on the claims. Is there an effective date on this?
