Can anyone tell me if the following counseling documentation would support counseling requirement for time based coding? Is this specific enough? Do we have to be able to decipher what is goal for maximal effort?

That doesn't sound sufficient to document the content of the counseling. It also sound like something that would take no more than 5 minutes maximum.
