Would you just bill hardware removal 20680? Or add something else? Dr. wants to add 27405(knee capsule repair) which doesn't seem right, but not sure what else to bill if anything. Thank you for any input! "Attention was turned to left proximal tibia. Prior incision was opened and the site of the seroma was sharply opened proximally over the incision. There was abundant mucinous, white material that was removed and sent for culture. All of this was excisionally debrided with a Roger and curettes and the lying of this seroma was also sharply debrided with a scalpel. The anterior compartment was elevated from the plate and 14 screws were removed in total with the plate. The joint was then evaluated closely and there was noted to be a large posterior lateral bone void at the site where the previous Progenix Plus bone void filler was placed at her 1st procedure. This clearly had not remodeled with mature cancellous bone. This cavity was evacuated with curettes and then copiously irrigated and aspirated. Given her inability to fill her bone void previously with void filler, decision was made to place antibiotic cement, which would also provide the additional benefit of immediate stability for weight bearing. High viscosity, gentamicin-containing cement was mixed under vacuum and then impacted under finger pressure into the void of the proximal lateral tibial plateau. The capsule was then imbricated laterally with #2 Ethibond. The small portion of inferior soft tissue void not filled with cement was then filled with mobilized anterior compartment sutured with 0 PDS to the anterior periosteum. A medium Hemovac was placed just under the anterior compartment for postoperative drainage. A layered, watertight closure was performed with multiple 0 PDS, #2 Ethibond proximally/superficially to the area of the prior seroma and then 2-0 Monocryl 1st interrupted and then in running locked fashion along the entire length of the fascia..."
