I bill for a radiology group, and we are pretty stumped as what to do with this one!

Report as follows:

MR Lower Leg Right WO Contrast 73718

PROCEDURE: MR Lower Leg Right WO Contrast 73718 CLINICAL INDICATION: EVALUATE RT PLANTARIS LONGUS
TENDON FOR FUTURE SURGERY PLANNING OF RT INDEX FINGER TENDON RUPTURE)

COMPARISON:None

TECHNIQUE: Limited MRI of the lower extremity/ankle axial PD and sagittal PD fat sat are obtained.

FINDINGS: The plantaris tendon is noted along the medial aspect of the Achilles and inserts along the medial calcaneus. The remaining visualized tendons and muscles are grossly unremarkable. No suspicious osseous abnormality.

IMPRESSION: Plantaris tendon is noted along the medial aspect of the Achilles and inserts along the medial calcaneus.

My gut instict is there is NO WAY Medicare is going to pay this for a hand tendon injury, but is there some other code (T code maybe) that would justify this? Our assumption is they are going to do a transplant of some type. This is one place where the info we have is REALLY lacking, and most of the time we kind of have to "wing it", this one just has us stumped!

Any ideas are greatly appreciated!
