one of my physicians performes thoracic outlet compression syndrome surgery. This surgery takes several hours and is very complex. During the surgery he removes the middle scalene and the anterior scalene muscles. The division of these muscles is included in the primary procedure but the removal of them is additional work. Can anyone suggest an appropriate code for the resection of these muscles?
 
Hmmm, what about 21700, if that doesn't work you could consider an unlisted code from the Musculoskeletal section OR think about adding a -22 if you just can't find anything that matches the work done.
