We are having discussion within our organization on how best to handle instances when radiology scan images are degraded by motion or artifact.
Can the hospital bill for this scan fully or should a reduced services modifier be used? These procedures do not involve anesthesia.

1. Patient had MRI brain wo; dictation states exam is significantly limited by motion artifact. How should this be billed by the hospital (technical)?

2. Patient had MRI brain wo; dictation states patient refused further scanning; only sag, dwi, and ax flair obtained. Limited study. No acute findings. How should this be billed by the hospital (technical)?

3. Patient had MRI Brain w wo; dictation states due to presence of braces, some of the sequences (DWI, SWI) are severely degraded. How should this be billed by the hospital (technical)?
 
Numbers one and three are easy; the full study was done.

For number two (without looking up all the required components of the study), I would say look at 76498 for unlisted MRI procedure, if you cannot find one that covers what was done.
